HL7 v2 is the event backbone inside most US hospitals — admissions, orders and results flow as pipe-delimited messages through an interface engine. FHIR R4 is the REST API standard for apps, patient access and cross-organisation exchange.

A realistic integration strategy consumes v2 feeds where they exist, uses FHIR APIs for app-level reads and writes, and normalises both into one internal model so product teams never touch raw interface formats.
